Wellbeing Assessment

A wellbeing assessment is a way of finding out what help you may need.

We will meet with you to talk about:

  • your needs, and
  • what matters to you.

You can have other people with you when we complete your wellbeing assessment.

We will ask you question like:

  • what matters to you?
  • what is happening in your life right now?
  • who supports you at the moment?
  • what are you good at?
  • what do you need help with?
  • what skills would you like to learn?
  • what would you like to happen next?

We will write down all the information about you and your needs.

We will agree a care plan with you.

Your care plan may include:

  • what matters to you and your aims,
  • what support you need.
  • things you can do for yourself,
  • support you get from your family and friends,
  • support that you can get in your community, and
  • support offered by the council
