Wellbeing Assessment
A wellbeing assessment is a way of finding out what help you may need.
We will meet with you to talk about:
- your needs, and
- what matters to you.
You can have other people with you when we complete your wellbeing assessment.



We will ask you question like:
- what matters to you?
- what is happening in your life right now?
- who supports you at the moment?
- what are you good at?
- what do you need help with?
- what skills would you like to learn?
- what would you like to happen next?
We will write down all the information about you and your needs.
We will agree a care plan with you.


Your care plan may include:
- what matters to you and your aims,
- what support you need.
- things you can do for yourself,
- support you get from your family and friends,
- support that you can get in your community, and
- support offered by the council

The Social Services and Wellbeing Act 2014 sets out how services in Wales should be delivered.
It tells us when and how assessments should be completed.
