Transition is a time in your life when you are growing up and getting ready for adult life.
Transition planning: leaving school
Transition planning meetings are held at school.
In this meeting you will:
- talk about your future, and
- write a plan saying what will happen next.
You can find out more about transitioning by watching the video.
There are different options you can choose when you leave school, like:

- support through social or health services.

College courses will be talked about in school.

Our local college is Cardiff and Vale college.
They have lots of exciting courses.
They will talk to you about your needs and look at how they can meet them.

You will have a transition plan to help you go to college.

The council team will work hard with the college to adapt courses for your needs
If the college cannot meet your needs, your education advisor will help you look for other colleges.

If you are interested in further education, education specialists will make the final decision with you.
Social workers will support you around your social care needs.

If you have a social worker they will refer you to Adult Social Services.
If we are the right team for you, a transition social worker will get to know you and find out what matters.
